Integrated Aged Care Assessment Services

 


The Integrated Aged Care Assessment Service is a program of Ovens and King Community Health Service Inc.  Ovens and King CHS provide support, administration and management of the service.

The Integrated Aged Care Assessment Program delivers services through two streams:

  •  Hume Aged Care Assessment Service (ACAS), Wangaratta
  •  Allied Health Services – Continence Nurse Advisor & Occupational Therapy.

The Integrated Aged Care Assessment Program is a community based program which aims to support older people and people with disabilities to maintain or increase their independence and remain connected to their family and community.

The Hume Region ACAS Wangaratta service is based in Wangaratta, with an outreach service based in Wodonga.  It provides Aged Care Assessment to the Alpine, Benalla, Indigo, Mansfield, Towong Wangaratta & Wodonga Shires.     The service is provided by a multi disciplinary team comprising Registered Nurses, Occupational Therapists and Social Workers.

Their role is to comprehensively assess the needs of older people and to assist them to access appropriate care and support.  They determine and approve eligibility for Commonwealth subsidised residential care, community aged care packages (CACP) and extended aged care at home (EACH) packages.

This team also provides Occupational Therapy and Continence Nurse Advisory Services to the Alpine, Benalla, Mansfield and Wangaratta shires.  These services are based in Wangaratta with an outreach Occupational Therapist based in Mansfield. These Allied Health services are targeted to the frail aged, the younger people with disability and their carers with the aim of providing specialist assessment and interventions to optimise the person’s capacity to remain at home and living as independently as possible.  The Continence Service provides additional services through the Continence Support Scheme providing aids and equipment, continence assessment and ongoing management support for 18 clients between the ages of 5 – 16 years in the designated Local Government Areas.
